If the office is running more than 10 minutes behind schedule, we will make every effort to contact you to let you know, as we understand that your time is valuable. Likewise, if you will be late, we would appreciate a phone call, keeping in mind that if you are more than 10 minutes late, your appointment may have to be rescheduled.
Payment is expected the day of service. We accept cash, check, Visa, Master Card and Care Credit.
If you experience a dental emergency, please call the office. Dr. Kozicki’s phone number is in the message. When we are closed for vacation, the phone number of a dentist covering for emergencies will be available on the answering machine. You can press 0 to bypass the recorded message and leave a voice mail right away.
Due to the many changes in insurance policies, it is no longer an easy task to interpret each individual policy. Although we try to stay aware of changes, it is not always possible. It is your responsibility to know your individual coverage. Failing to know could result in you being responsible for costs incurred. Please remember, your insurance policy is between you and your insurance company and employer. Although we bill your claims for you, it is ultimately your responsibility for payment of services rendered.
